2. All Assist Plans require an intake form so it may be confirmed that the best plan was selected for your needs while also gaining a clear picture of your unique requirements.

3. Consultations are required for Pro-Hand Assist, Green-Hand Assist and Super Green-Hand Assist. In some instances, a consultation can be required for Right-Hand Assist - all are welcomed to schedule a consultation, however.
